Collection Kit Contents

  • Sealed Urine Collection Cup. Do not use if the seal is broken.

  • Alcohol Wipes: For sterilizing and cleaning.

  • Personalized Barcode Label: Unique to you and your sample. Attach it as directed.

  • Small Marker: For labeling

  • Delphistem supplement tube. To be mixed with your sample upon collection to ensure sample vitality.

  • Delphistem Stickers: Yours to keep!

Instructions for use

  1. Open Container: Use the provided sterile container.

  2. Collect Urine Sample: Fill the container to approximately 80% capacity.

  3. Delphistem supplement: Add the contents of the delphistem supplement tube into the container.

  4. Secure Container: Close the container tightly to prevent leaks. Flip once to ensure sample and supplement are mixed.

  5. Clean with Ethanol Wipes: Clean the exterior of the container using provided ethanol wipes.

  6. Date and time: Write (i) the collection date and (ii) hour directly on the container label using the provided small marker. (No other information is needed.)

  7. Barcode label: Attach the provided barcode on the closing cap.

  8. Pack for Shipping: Place the container securely back in the provided box for shipping. Ensure the container is firmly closed.

Once your sample is ready for shipping, use the prepaid post label provided.

Do not expose sample to extreme temperatures (ideal range 55F-85F/ 12C-30C)
